Dear Returning Student-Athletes and Parents,
Welcome back to the University of Minnesota! The Athletic Medicine Unit is committed to providing our student-athletes with the best possible medical coverage. Please take the time to review the policies and procedures regarding athletic injuries and medical insurance. We continue to be available to assist the student-athletes and parents with any questions pertaining to medical services and insurance requirements.
Returner's Pre-Participation ProcessPlease print out the following form, complete it and return it to Deb Hoffoss at the address below on or before August 1, 2017 (clicking on the name of the form will take you to the form):
- Demographics/Insurance Information Form *Please include copies of the insurance cards for health, dental and pharmacy (front and back).

Please contact your sport specific Certified Athletic Trainer to arrange a pre-participation annual medical history and health appraisal.
We encourage you to discuss your health plan coverage with your parent(s) prior to the start of the year and obtain an updated insurance card to carry with you at all times.
